Patients often point to the lines that bother them in the mirror and say, “Can you just fill this line?” The most common culprits are nasolabial folds and marionette lines. While we can soften those lines directly, modern facial aesthetics starts by asking why they formed in the first place. The answer, more often than not, is midface volume loss.
Why Nasolabial Folds Deepen with Age
As we age, several things happen at once:
- The fat pads in our cheeks descend and deflate.
- The bony support of the midface changes.
- Skin loses elasticity and drapes differently.
All of this makes the fold from the side of the nose to the corner of the mouth, the nasolabial fold, look deeper, even if we haven’t “added” anything there. If we only chase the fold itself with filler, we risk heaviness and an unnatural look.
The Role of Cheek Volume and Structure
In many patients, the first step in softening lines around the mouth is restoring gentle support to the cheeks and midface. With carefully placed filler:
- The midface gains lift and contour.
- The overlying skin is better supported.
- Nasolabial folds and marionette areas naturally appear softer.
This doesn’t mean “big cheeks.” It means subtle restoration of the youthful scaffolding that quietly supports the lower face.
When We Treat the Lines Themselves
There are times when we do treat the folds and marionette areas directly, usually:
- After we’ve addressed midface support.
- In very thin patients with etched lines.
- When folds are out of proportion to the rest of the face.
In those cases, I use softer, more flexible fillers and conservative amounts to avoid puffiness. The goal is to blur the transition, not erase every line, which would look unnatural, especially when you smile.
What a Typical Midface Filler Plan Looks Like
A thoughtful midface treatment may include:
- One or two syringes of structural filler in the cheek area.
- Optional micro-dosing around the nasolabial or marionette regions.
- A staged approach over one or two sessions rather than doing everything at once.
