Beauty Banking Terms

Read these terms with the completed billing schedule and payment authorization supplied at enrollment.

Membership Summary

Monthly contribution
$99 each month. The full $99 becomes $99 of Purchased Beauty Bank Balance.
Enrollment fee
A $50 fee applies when joining or rejoining; it is not banked. The amount charged is recorded at enrollment. Non-refundable except as provided in these Terms or required by law.
Active-member benefits
10% off eligible regular-price services, including injectables; 15% off eligible regular-price in-spa products; one 20- or 30-minute Pure LED treatment per paid membership period.
Renewal and cancellation
Month-to-month; automatically renews until canceled. No minimum term or early-termination fee. Cancel before the next scheduled charge. Allow up to two business days for manual confirmation; see Section 5.
Your paid balance
Does not expire or disappear upon cancellation. After membership ends, use it at then-current nonmember pricing, subject to the eligibility rules below. Cash refunds only as described below or required by law.
Important exclusions
No discount stacking, gift-card purchases, or discounted prepaid packages. Online/affiliate-store purchases are excluded. Unused monthly LED benefits do not normally roll over.
Taxes
Applicable taxes, if any, are additional and disclosed before charging. Taxes and enrollment fees do not become Beauty Bank Balance.

1. Parties and Purpose

This Agreement is between you, the member named on page 1, and Vivian Chin, MD, PLLC, doing business as Koru Wellness Aesthetics (Koru). Responsible licensed clinicians retain independent medical judgment. Beauty Banking combines prepaid purchase credit and membership benefits; it is not insurance, a loan, or an interest-bearing savings account.

2. Purchased Beauty Bank Balance

Each successfully collected $99 monthly contribution creates $99 of prepaid credit (Purchased Beauty Bank Balance). Enrollment fees and taxes are separate. There is no borrowing against future contributions. Additional payments do not create additional membership benefits unless separately agreed in writing.

Purchased Beauty Bank Balance does not expire, decrease through inactivity, or become forfeited when membership ends. No dormancy or balance-maintenance fees apply. Legally required unclaimed-property treatment remains applicable; Koru will not retain unclaimed funds as forfeited.

Apply this balance at face value to eligible Koru services and in-spa products, excluding gift cards, membership/enrollment fees, discounted prepaid packages and online/affiliate-store purchases. Further exclusions require advance disclosure under Section 10. Applicable law controls restrictions on purchased credit.

An active member receives the applicable eligible discount first, then applies available balance as payment. Using prepaid balance is not an additional discount. Any unpaid remainder is due at the visit. Once membership ends, remaining balance is redeemable at then-current nonmember pricing; continued membership is not required to spend it. Requests for cash or card refunds are governed by Section 6 and applicable law.

Koru will track purchased balance separately from complimentary promotional credits and monthly service benefits. Promotional-credit terms, including any expiration, must be disclosed when granted; such terms do not apply to purchased balance. The program does not guarantee future promotional offers. On an otherwise authorized return or refund, value paid with purchased balance returns to that balance unless the law requires another remedy.

3. Active-Member Benefits and Eligibility

During each paid membership period, members receive 10% off eligible regular-price services, including injectables, and 15% off eligible regular-price in-spa products. Discounts use the regular price in effect when the purchase is made, not a permanently locked treatment price. Enrollment must be completed before a service is performed or a product is purchased; discounts are not retroactive.

Discounts do not combine with packages, sale prices, other membership discounts, gift-card purchase promotions, or other offers unless Koru expressly authorizes a combination. Eligible manufacturer rewards may be used only where the current rules of the relevant program permit. Koru does not control or guarantee those programs. Membership benefits are personal and nontransferable; any transfer of purchased balance requires Koru approval unless otherwise required by law.

One complimentary 20- or 30-minute Pure LED treatment is available per paid membership period, with the appropriate option determined at booking and by suitability. It is subject to appointment availability and has no cash substitution or guaranteed retail value. Unused benefits ordinarily expire at the end of that period. If Koru cancels an appointment and cannot offer a reasonable replacement within the period, Koru will arrange a carryover appointment or another agreed remedy.

Appointment policies disclosed before booking still apply. Koru will not both impose a missed-appointment fee and forfeit the covered treatment for the same missed appointment. Any reservation credit already paid is applied toward an applicable fee first; the same fee is not charged twice. No separate debit from purchased balance is authorized by this Agreement alone.

4. Billing Schedule and Payment Safeguards

The completed page 1 schedule governs billing. Koru discloses the first contribution date, initial paid period, next charge date and recurring billing day before consent. If a billing day does not occur in a month, use the last day. No additional first-of-month schedule applies. A shortened initial period must be disclosed; the contribution still adds the full $99 purchased credit.

Koru will provide a retainable agreement and confirmation of the charge amount, frequency, cancellation methods and next billing date. Keep contact and payment details current. Enter payment credentials only in the payment system. This authorization does not permit unrelated treatment charges, penalties or debits from purchased balance.

If a recurring payment fails, no balance or new-period benefits are created by that failed payment. Koru may notify you and make reasonable retries of that unpaid installment while authorization remains in force. Koru will not continue retries after authorization is withdrawn or initiate future renewal charges after an effective cancellation request. Any retry must not duplicate a payment already collected. No separate late or collection fee is imposed by this Agreement.

Koru may correct documented system or clerical errors with notice and an explanation. If a contribution is refunded or finally reversed, only the corresponding funded amount may be removed; there will be no double deduction. During a genuine payment dispute, Koru may temporarily restrict only the disputed, unspent credit as reasonably needed, not confiscate unrelated paid funds. Account adjustments will follow the outcome and applicable law. Nothing limits your right to dispute a charge with your payment provider or creates authority to charge your card for a contested shortfall.

5. How to Cancel

Cancel without a reason, approval or early-termination fee: use the online link supplied at enrollment, email hello@korunyc.com, call 212-396-3672, visit in person, or mail Koru Wellness Aesthetics, 341 East 78th Street, New York, NY 10075. Ordinary mail is sufficient. Cancellation is also accepted through any medium offered for enrollment consent.

Cancel before your next scheduled charge is processed to stop renewal. For manual requests, please contact us two business days ahead whenever possible; allow up to two business days for confirmation. Administrative business days are Monday, Wednesday, Thursday and Friday, excluding holidays. Requests are recorded when received, including on closed days, not when staff process them. Confirmation is not a condition of cancellation. Processing time does not extend billing authorization. A renewal charge processed after a timely request will be corrected and refunded. Confirmation includes the benefit-end date and remaining purchased balance.

Ordinarily, cancellation stops future renewals immediately and benefits continue through the end of the current paid period. Purchased balance remains available afterward without membership benefits. Not attending appointments does not by itself cancel recurring billing. No automatic freeze or pause is included; any separately agreed pause must state its billing and benefit effects in writing. Rejoining requires a new agreement and the enrollment fee disclosed before enrollment.

6. Refunds and Mandatory Cancellation Rights

Ordinary voluntary cancellation does not require a prorated refund for the current paid period or a cash refund of purchased balance. Unused purchased balance remains redeemable. The enrollment fee is not banked and is non-refundable except where this Agreement or law requires otherwise. Past member discounts are not retroactively charged back merely because you cancel.

All mandatory cooling-off, renewal-cancellation, disability, relocation, military, death, refund, and other consumer rights remain in force. A mandatory right overrides any conflicting fee, deadline, non-refund provision, or other term. Koru will process a legally required refund within the applicable deadline and honor any required small-balance cash redemption.

7. Clinical Judgment and Separate Treatment Consent

Membership guarantees no particular procedure, product, clinician, appointment, treatment frequency or result. Treatment requires assessment, medical history review, consideration of anatomy and goals, contraindication screening, informed consent and independent clinical judgment. Koru may recommend no treatment, another approach or postponement despite available balance.

This Agreement is not consent to a medical procedure and does not replace treatment-specific consent, privacy notices, or clinical instructions. It does not waive claims for professional negligence or other nonwaivable rights. Membership enrollment, payment authorization, treatment consent, and marketing or photo permissions are separate decisions. You may use retained balance for another suitable eligible purchase; where no reasonable eligible purchase can be provided, Koru will review return of unspent purchased funds and any legally required remedy.

8. Conduct, Termination and Program Closure

Koru may decline renewal or terminate benefits for nonpayment, documented fraud or misuse, unsafe or abusive conduct, material breach, or discontinuation of the program, subject to applicable law. A good-faith billing dispute alone is not grounds for confiscating funds. Koru will give notice, stop future recurring charges, and state the status of purchased balance and any refund.

Termination of benefits does not forfeit purchased balance. If Koru terminates the relationship and will not permit you to redeem eligible purchased credit, Koru will refund the unspent purchased balance, subject to lawful offsets for documented unpaid purchases and applicable law. If the program ends but Koru continues operating, the balance remains redeemable without membership. If Koru permanently closes or cannot honor the balance, it will return unspent purchased funds as required by law and this Agreement. Any adjustment must avoid double recovery.

Membership cancellation is separate from termination of an established clinical relationship. Clinical discontinuation remains subject to professional duties, appropriate notice, and any continuity-of-care obligations. Temporary schedule changes or closures will be communicated when practicable; they do not authorize continued billing for a materially unavailable program contrary to law.

9. Membership Records and Communications

Koru provides balance and transaction history on request and retains the agreement version, authorization, billing notices and cancellation record. Report suspected errors promptly; delay does not waive statutory or payment-dispute rights. Reasonable identity checks must not unnecessarily obstruct cancellation.

Program notices may be provided to your designated email address, with a paper copy available on request. Communications about billing, membership administration, and cancellation are distinct from consent to marketing. Contact Koru to update your notice preferences or address. Do not send full payment-card numbers or security codes by email.

10. Prospective Program Changes

Material changes to benefits, exclusions or other terms take effect prospectively after written notice, normally 30 calendar days before the stated effective date, subject to legally required timing and methods. Notice explains the change, effective date and cancellation process. A revised web page alone is not notice or consent.

Koru will obtain your affirmative consent before increasing your recurring monthly contribution or changing its frequency. Without that consent, Koru will either continue the authorized arrangement or give notice ending future renewal; it will not silently charge the higher amount. Ordinary service and retail prices may change, but members receive a price quote before purchase and the applicable percentage discount. Purchased balance already funded will not be reduced retroactively by a program change.

11. Governing Terms and Dispute Resolution

New York law governs this Agreement, subject to applicable federal law and nonwaivable consumer protections. The completed Membership Summary, these Terms, and any expressly identified, approved addendum form the membership agreement for this enrollment. An advertisement or later website edit does not silently amend it. Treatment-specific consent documents address the clinical services to which they relate.

Contact hello@korunyc.com or 212-396-3672 about a membership concern. Informal review does not limit access to a regulator, a payment provider, or any legally available forum or remedy, and does not require you to delay a time-sensitive claim. An invalid term does not invalidate the remainder where lawful. Failure to enforce a term on one occasion does not waive it for the future.

Enrollment and Payment Authorization

The member accepts the agreement, and the actual payer/cardholder separately initials the recurring-payment authorization and signs the payer section, even when the member and payer are the same person. The completed agreement records the billing schedule and online cancellation link. No signature or payment is collected on this information page.
